- If a completed assessment indicates that you should pursue outpatient , intensive outpatient , or partial hospitalization treatment with The Light Program, we can admit within 24-48 hours, or potentially the same day, depending on the next scheduled session day.
- If our assessment determines that you require a higher level of care than we offer, we will refer you to our network of trusted partners.

The Empire Plan And Medicare+choice Hmos
You may receive information from Medicare and from HMOs in your area describing Medicare options available to you that are not part of NYSHIP. You may wonder whether to join one of these plans to supplement or replace your Empire Plan coverage.
Please be aware that your Empire Plan benefits will be significantly reduced if you join one of these plans. Under a Medicare+Choice contract, you assign your Medicare benefits directly to the HMO. You give up the right to Medicare coverage for services outside the HMO. All benefits must be received from that plan, even if you also have Empire Plan coverage, for example, through your spouse.
If you cancel your NYSHIP coverage, you lose several benefits:
- Your former agency is no longer required to reimburse you or your Medicare-eligible dependents for the Part B premium.
- If your former agency allows your sick leave credit to be used to reduce your premium, it will no longer do so.
- If you wish to re-enroll in NYSHIP, you will be subject to a three-month waiting period before your coverage again becomes effective.
- If you die while you are not enrolled in NYSHIP, your dependents are not eligible for dependent survivor coverage.
If you are enrolled in the Empire Plan and you or your enrolled dependent also joins a Medicare+Choice HMO, the HMO coverage will replace your original fee-for-service Medicare coverage and drastically reduce or eliminate benefits available under the Empire Plan.

In light of the current COVID-19 outbreak in New York State, CSEAs Joint Committee on Health Benefits has worked with New York State to offer a pilot Telemedicine Program for Empire Plan enrollees/dependents to begin immediately.
This voluntary Telemedicine Program, through LiveHealth Online, allows Empire Plan enrollees/dependents access to a live doctor 24 hours a day, 7 days a week without leaving their home . Covered-in-full services are available for medical as well as mental health visits through Dec. 31, 2020.
If you are enrolled in the Empire Plan, you will be receiving a postcard from New York State describing how to use this benefit by using an electronic device such as a laptop, tablet or smartphone. This telemedicine program is provided by LiveHealth Online claims for the telemedicine visits will be processed by Empire BlueCross, the Empire Plans hospital carrier.
Empire Plan enrollees/dependents who require immediate access to medical or mental health telemedicine services can visit Empire Blue Crosss website and register through the LiveHealth Online link. To receive paid-in-full benefits, the Empire Plan specific coupon code must be added when registering for medical telemedicine services or when receiving mental health telemedicine services.

- submits claims directly to UnitedHealthcare for covered services or supplies you receive and

When you use a non-participating provider, covered benefits are payable under the Basic Medical portion of the Plan, but using a participating provider lowers your out-of-pocket expenses. The principal health insurance plan in New York is The Empire Plan, also known as NYSHIP . It is the largest public employer health insurance program in the U.S., with over 1.2 million members.
This program, created decades ago, offers an impressive range of health plans to both state and local government employees. Not only are active employees eligible for coverage, but so are retirees, dependents and dependent survivors. However, the program was not always so versatile. When NYSHIP was first founded in the late 1950s, coverage only extended to state employees. The Empire Plan Expands Use Of Unitedhealthcare Network
The Empire Plan members have nationwide network access, starting Jan. 1, 2021.
Starting Jan. 1, 2021, members of The Empire Plan will have in-network access to the UnitedHealthcare Options preferred provider organization network for all states and U.S. territories outside of New York.
The Empire Plan Supplement will be included with the 2021 UnitedHealthcare Care Provider Administrative Guide. When the guide is posted, the supplement will apply to care providers in all markets outside New York, as well as national care providers and a limited number of providers in New York with The Empire Plan specifically added to their UnitedHealthcare Participation Agreement.
Identifying members of The Empire Plan
Members are New York State government employees, local government employees with Empire Plan or Excelsior Plan benefits, graduate student employees in the Student Employee Health Plan , retirees and their dependents. Empire Plan benefit ID cards have New York State Health Insurance Program and The Empire Plan on the front of the card. Recent cards also have the UnitedHealthcare logo. Cards arent replaced each year and some valid cards may look different.
