In recent decades in the United States, discussions around mental health struggles have become less stigmatized. A study in the journal, Health Affairs, found that this is likely due to a combination of increased media exposure, increased employer recognition of these issues, and the continued efforts of public education programs conducted through mental health organizations. However, the same study noted that while mental health as a subject has benefitted from more awareness, many Americans face a lack of resources for treatment particularly due to the immense deinstitutionalization that has occurred over the last several decades.
At United Way of the National Capital Area, we are aware of how the issue of mental health intersects with other social issues, including race, poverty, homelessness, domestic abuse, and more. For example, according to one study of adults with a diagnosis-based need for mental healthcare, 37.6% of White people received treatment, compared to only 22.4% of Latinos and 25.0% of Black people.
Part of our health initiative is to bring these inequities to light and continue to discuss them. One area we wanted to focus on was the accessibility of treatment in the mental health sphere. With so much of mental health and treatment changing due to COVID-19, and in honor of Minority Mental Health Awareness Month in July, we wanted to conduct a data study of mental healthcare access and attitudes across the United States. Below are our full findings.
Best And Worst Mental Health Care Rankings By State
Mental heath needs to be cared for just like physical health, but there’s unfortunately stigma that prevents a lot of people from tending to their mental health. In fact, a new study found more than half of people who need mental health treatment do not get it. That’s a crazy statistic, but it could have something to do with the dismal availability of treatment providers that varies greatly from state to state. According to the study, states like Nevada and Arizona have very few providers and a poor rate of people who need treatment getting it, making them the worst places to live if you have mental health issues.
Mental Health America’s annual State of Mental Health rankings found Nevada is the worst place to live for access to mental health care, while Connecticut is the best.
Nevada bottomed out the list because few people who need mental health treatment get it and there’s a shortage of providers. Alabama, Arkansas, Idaho and Arizona rounded out the bottom five. Alabama has a scary shortage of providers, with only one per 1,200 people according to the report.
The best states according to the report are Connecticut, Massachusetts, Vermon, South Dakota and Minnesota. These states have more mental health providers and the people who need treatment are more likely to get it.

– Suicidal ideation is increasing amongst adults. More than 10 million adults are estimated to have had serious suicidal thoughts a jump of more than 450,000 people from last years data.
– Drug and alcohol use and abuse vary greatly by state. For example, prevalence of substance use disorder ranges from 6.32% in Georgia to 11.55% in the District of Columbia. It did however decreased slightly, nationally, from 8.46% to 7.68% between 2012 and 2017. But use of substances prescription, legal and otherwise is highly correlated with mental health illness for adults and young people.
– Although more Americans are insured, coverage is proving deficient. Unmet need for mental health treatment in youth and adults is still lacking. More than 10 million adults reported an unmet need for care. Further, it was estimated that the proportion of youth who had private insurance, but were not covered for mental or emotional difficulties, almost doubled.
– In contrast, Wyoming, Utah, Idaho, Oregon, and Nevada were rated lowest in the overall state rankings. For adults only, these states remained consistent. Youth were slightly different however, adding Alaska and South Carolina to the bottom five states for mental health.
Improving Access To Care Requires Strengthening Primary Care And Extending It To Every Local Community
Access to care, however, requires more than insurance coverage. Convenient and timely primary care is also vital. Top-ranking countries like the Netherlands and Norway ensure timely availability to care by phone on nights and weekends . In the Netherlands, cooperative GP posts are staffed by general practitioners , who are obligated to provide at least 50 hours of after-hours care annually in order to maintain their professional licensure. In Norway, the Patients Rights Act specifies a right to receive care within specific timeframes and with maximum wait times applying to covered services, including general practitioner visits, hospital care, mental health care, and substance use treatment.
In top-performing countries, workforce policy is geared to ensuring access within communities, especially those that have been historically marginalized. Norway, with the highest number of doctors per person among the 11 countries in our study, has a much larger supply of physicians relative to its population than the U.S. has. Outside the U.S., a larger proportion of clinicians are devoted to primary care and are geographically distributed to match population needs. For example, Norwegian local municipalities, which are responsible for the supply of GPs, may apply to the national government for extra funding to ensure they have an adequate number of physicians.
